This tablet is manufactured by Breckenridge Pharmaceuticals and they list it as containing 5mgs of Cyclobenzaprine, which is a muscle relaxant. The FDA lists its typical side effects as possibly including nausea, dizziness, headache and dry mouth.

If it is in a bottle as dispensed by your pharmacy, it expires one year from the date it was dispensed. If it is still in the original factory sealed bottle, then it should be good for about 4 years.
