Hello, Nannyq! How are you? I'm sorry that you're in pain.

However, due to the new regulations that were set forth by the DEA last year, you'll need to see specialists, if you require such medications on a long-term ongoing basis, a general practitioner won't be able to prescribe them for you. That also means most doctor's won't prescribe both, you'd need to see a pain management specialist and a psychiatrist for the Xanax.

The FDA lists Xanax as a benzodiazepine that is most commonly used to treat anxiety and nervous disorders. It carries the risk of being habit forming and could cause side effects, such as nausea, dizziness, headache and irritability.

Can anyone recommend good doctors in that area?

I don't know the law in Georgia but if we're talking about the DEA, that would be federal law and you're all wrong once again. I live in Michigan and my primary care doctor prescribes two strong opioids and Xanax for my serious chronic pain. (The Xanax also serves as an adjunct medication to help me sleep since I also have a terrible sleep disorder.) I do not see, nor do I HAVE to see a pain specialist. My current doctor has been prescribing these to me for seven years; prior to that, my primary doctors in two other states also prescribed these same medications. (We've made two moves in the last 17 years which accounts for the switches in my PCP's.) I see my doctor every three months and he's never said anything about now being required by the DEA to see a pain specialist for these medications nor has the pharmacy which I've used for the last seven years. Before you shoot off statements as "facts," please make sure they are indeed facts and not just rumors or, perhaps, the way you THINK things should be done.

By the way, I have been to pain specialists in the previous two states where we lived and they were both a disaster. My illness is severe, well-documented and has been firmly diagnosed by two rheumatologists. Yet both these pain "experts" blew me off. One said I probably just wanted attention and wouldn't do anything for me. The other said he couldn't help me, even though he claimed he couldn't find any of the information that had been sent by my PCP (which meant he had no information on my case.) He also didn't ask me any questions about my illness. The fact that he had no information on my case didn't stop him from claiming that this pain clinic didn't treat my particular illness, even though it's primary symptom is pain. On the way out, I told the person at the front desk that the doctor had gone looking for my information folder during my appointment and had returned claiming he couldn't find it. I told her that I had checked with them the day before and been told that they had received all my information from my primary doctor. She reached behind her, glanced at the materials on the table there and handed me all my information, organized in one folder (yes, the information the great pain specialist said they didn't have and couldn't find) and told me I could "just keep it." This doctor also mentioned that he couldn't do anything about my "back problem"--which wasn't surprising since I don't have a back problem and never have and, therefore, wasn't there to see him about non-existent back pain. When I stated that I didn't have a back problem and asked where he was getting his information about my condition, he just kept repeating that he couldn't do anything for me. There was an older doctor observing this guy at the back of the examining room so the "doctor" I saw was probably an intern or resident. The supervising physician at the back of the room never said a word. After these experiences, I won't be looking to any "pain specialist" as an expert on treating me. And, I repeat, I'm getting opioids and Xanax from my family practitioner with no end in sight so stop pushing this incorrect information that these meds have to be prescribed by a pain specialist.

By the way, I also saw a "sleep specialist" about my sleep disorder. She had me undergo a sleep study which showed no problems with apnea or anything else (other than that I wasn't sleeping). Then she kept me coming back for appointments every few weeks; all she did at these appointments was ask me how I was sleeping now. Since she wasn't doing anything to treat the problem, I'd just tell her (AGAIN!) how badly I was still sleeping. She'd then have me make another appointment for the next month and send me home. When I finally confronted her (courteously!) about this process, she said I was being a difficult patient. Needless to say, I fired her on the spot. Quite the money making scam she had going. My current family medicine doctor worked with me for the first few months I saw him, trying some different things. We finally hit on a combination of medication and sleep process that works better than anything else I've tried. As far as I'm concerned, pain and sleep specialists are highly overrated; they're just more expensive.

A family Dr can prescribe pain meds and benzodiazapines up to a certain amount. My Dr prescribes Percocet 10/325 3 times daily. That's 90 per month. He said any more than that would be considered Pain Management.

I'm prescribed 4 - 60 mg Oxycontin and 8 - 8 mg Dilaudid per day for severe, 24/7 pain by my PCP. That's 120 of the Oxy and 240 of the Dilaudid, all from my PCP, no pain "specialist" involved.

Any dr can prescribe any meds they choose to. But, unfortunately many tell their patients that laws prevent them from prescribing certain meds or certain amounts of meds. The person that says that due to new regulations you have to see a certain dr to be prescribed a certain med must have been lied to by their dr. Although many PCP's are choosing to not prescrbe certain meds, it is not a law that is preventing them from doing do but their own individual choice. There are also no laws that state you must have a drug panel done or sign a pain contract, but many drs tell their patients there are. Ask for a reference in the law when you dr tells you that something is Law and look it up online or at your local library. If they are lying, they won't have it or will give you one that does not apply to what they said.

They aren't actually regulations, I think that was a bad way of wording. They are actually just directives from the DEA, to try to curb over prescribing and improper prescribing.

Doctors that don't abide by them are going to face a lot more scrutiny, so many don't feel it's worth the risk.

I have to agree with those who side the GP can prescribe whatever he wants, getting it filled is the hard part. The big chain pharms (CVS, Walgreens) generally will not fill if a GP, only pain management docs. My GP (Florida) has been writing me Xanax 1mg. 2/day for many years and was until recently writing for Norco as well. Since the new laws I can't get Norco's filled anywhere so we switched to Gabapentin and generic Tylenol 4 at 4/day. It's not the same but it helps. When I get insurance I'm going to a pain doc and see if he will prescribe generic oxy's (no tylenol) like my wife gets for her degenerative disc disease. In the meantime my Xanax, Tylenol 4, Soma's will have to do for my herniated discs and arthritis until I can afford insurance. Tough times and the D.E.A. (Drug Eradication Agency) is hurting us as much as our physical symptoms.

It may vary from pharmacy to pharmacy but I've filled two separate strong opiate prescriptions for a high number of pills at Walgreens, RiteAid, CVS, and 2 independent local pharmacies. This is over a period of several years since "pharmacy hopping" is not something I like to do. I fill my monthly prescriptions at the same pharmacy. But my usual pharmacy is at a Walmart-type store that isn't national so occasionally, if I'm out of town or out of state and need to fill my prescription for these pills, I have to fill them elsewhere. I have my doctor's understanding and approval if my prescription schedule requires this. The important point here is--ALL OF THESE PRESCRIPTIONS ARE WRITTEN BY MY PCP AND NO PHARMACY HAS EVER SAID THEY WON'T FILL THEM BECAUSE THEY'RE NOT WRITTEN BY A PAIN SPECIALIST.

I have degenerative disk disease among a host of other back problems from a car accident. I also have a problem with my hip being out of alignment which makes it worse. The shots don't help at all and actually have made things worse. I am in Florida. Coming from North Carolina, being on 10mg Percocet, Florida pain management put me on 7.5mg Loratab! They do not help! He does not listen to me and treats me like I'm an addict or something. This has been a 15 year ongoing physical problem. It's not just like I all of a sudden said, oh I have pain, give me drugs! I have plenty of records. My pcp told me to go to a dr in Another state but I dont know one and I dont want to keep changing drs and get accused if dr shopping. Does anyone have any good dr yhey can reccomend in Florida, Alabama, or Georgia?

What kind of pain do you have to get that many meds? What state do you live in? I wish I could find a doctor that had that much compassion.

I don't know the laws in the USA in regards to Xanax and pain medication or if any government agency monitors these types of prescribed medications. Here in Australia a Dr has to ring a government agency to get an authority number to prescribe Alprazolam (Xanax) and the Dr must state for how many and the number of repeats, depending on an individuals dose this agency monitors the authority numbers, by that I mean for example if you are prescribed Xanax 3 times a day it is averaged out and if you go back for more before this "averaged" time expires you don't get another authority, I have heard they do give you X amount of days grace period. If this does happen then they know the medication is being abused or even sold on the street and you no longer have regular access to the Xanax, it is only authorized for perhaps each script rather than 3 - 6 months worth, if that makes any sense to you.

As for pain relief I don't know much about them even though I am on endone. Dr's can prescribe that without any authority numbers from the relevant authority. There are prescribing guidelines in place and the endone that I am on, once again dose depending it is monitored and if something is suspected the pharmacist will refuse to fill the script and ask you to see you GP, the reason a pharmacist will refuse to fill your script is because of the information they have on file of your last script and the number of days it took you use the pain relief. Doctors here a pretty well on the ball in who is abusing the drug or the system as well and that's when they will refer you onto someone else.

Doctors here where I am, the older doctors will prescribe endone but the newer generation of doctors will not, apparently they are being taught at med school to veer away from any addictive pain medications, there are some young doctors that will prescribe pain relief because they are or have been made aware of your condition(s). I had to get a second opinion on a couple of matters and this doctor asked me if my GP was young I replied with the answer "Yes" he then shook his head and said I wish they would leave medications that are working alone, if everything is working then it shouldn't be fiddled around with, this doctor is now my GP. Does the USA have this sort of set up where the doctor has to call a government agency (medical) for authority numbers? I realize our medical systems are different and we have government assisted schemes for medications, in this case it's the government that says "NO" not insurance companies for those who are lucky enough to be in private health down here.

I believe at this point each state has set up its own laws on who can prescribe opiates and anti anxiety medications. And it matters the amount too. Here in Tennessee a general doctor can give you pain meds if you break your leg or have soke kind of accident but after a couple months at most they refer you to a pain clinic. The new FDA report does warn doctors about prescribing opiates and benzos together and so the doctors themselves here are changing the way the do things. PCPs are making patients go to psychiatrists to deal with it. Tennessee has set up a database of all patients on opiates which can be accessed by any physician you see or pharmacy you fill at. They know what you get where you get it and from whom. This database was set up last year. I think all the states are going to get to a similar point because of these reports coming out from government agencies. It is just some states implemented the crappy changes quicker. Tennessee is a red state big time and in the Bible belt I expected them to get crazy on us fast!
