I do not remember what this pill was for. I usually take generic brands of Zocor (20mg) and Zanex for anxiety. From what I've been able to find, this is an over the counter product called Guaipax SR that is listed as containing 75mgs of Phenylpropanolamine and 400mgs of Guaifenesin, it is a cold remedy that helps relieve nasal congestion. The FDA lists the typical side effects as possibly include nausea, dizziness, headache, dry mouth, dry throat, nervousness and insomnia. That also explains why it's been hard to find an identification for it, over the counter medications are not required to be listed with the FDA like prescription drugs.
