If the doctor has instructed him to take both doses, then he should do so. That blood pressure reading is very high and could place his life in danger, so the medication has been prescribed to try to bring it down as fast as possible.
The U.S. FDA lists its typical side effects as possibly including nausea, dizziness, headache, increased urination and hypotension.
