Updated

Just had Testopel pellets inserted in my backside. Forgot to ask doctor how long until they take effect. He's out of town.

Thanks Ginger, I just started on the T pellets and don't know yet if I am an aromatizer or not. My doctor is watching all my levels including estrodial, especailly since I voiced a concern about possible shrinkage and he assured me that we would keep an eye all my levels and address whatever we need to. I feel comfortable with him, but will always do research of my own to be able to have the knowledge I need to have an intelligent conversation with him and make a good decision about my body and health. Thanks again.

Your welcome Kyle. Hope it all goes well! As long as your estradiol to testosterone is 5% or less, DIM should be all that you need. Feel free to ask me anything, I really have studied this a lot and have read many studies on pellet therapy! The therapy I'm on is now in 23 states and the treatment lasts 5-6 months for men and they also look at your Vitamin-D and Thyroid.

Hi, i have had 5 testopel injections, for me they work better than anything else. Each pellet supposed to increase T level by 50 points so knowing your baseline is important to get to your magic level. For me that T level is over 500, which makes me feel 25 again. Doc use lidocaine and I ice the site for 20 minutes every few hours the first day to limit brusing. Doc also gives me cephalexin for 5 days to prevent infection. It lasts me 3 months then drops like a rock. The gels, lotions and transdermals dont work well for me. On Axiron I couldnt get my levels above 440 even with 4 pumps a day and too expensive. Injections never worked that great but that was 15 yrs ago. Hope that helps.

How does the Vit-D and Thyroid play along with low-t? All those have been low. Treated individually. Little relief. I'm understanding the collective balance is more important.

I have heard of the saliva test but where would you get something like that at?

Online, lots of places online.
Google, ZRT Saliva test

Frank, I think that your post is informative, but could be slightly misleading to some. By no means am I attempting to discredit your post, however, just a few notes
1). It can not truly be stated that implantation of each pellet raises T-levels by 50 points. For example, one individual may have an extremely low T-level of perhaps 50. The urologist may insert 12-14 pellets. At one month post procedure (When T-levels tend to be near their peak, for one person, they may elevate to 450, another 700, another 600, but estradiol levels may become significantly elevated (i.e.to say @ 200....... Excessive conversion of Testosterone to Estrogen. This is best treated with simple oral Arimidex)
2).A certain percentage of the population simply can not absorb topical transdermal preparations. Additionally it seems that if one is not favorably absorbed, none of them will be......regardless if applied to the inner thigh or under the arm.
3).As you have stated, to the opinion of some, there is at most minimal discomfort after the procedure. There is no pain with the procedure due to the administration of an anesthetic such as xylocaine. The wound may be closed with simple steri-strips. While the utilization of an antibiotic post procedurally can not really hurt, typically they are not necessary as infection rates should be virtually non-existent.
4).If the procedure is cost prohibitive, simple intramuscular IV's, typically of T-Cyprionate may be self administered. 5).Finally, one of the greatest challenges may be getting a PCP to agree to proceed with T-therapy. Some will not initiate unless levels are under 400, some 300. I would say that as long as there are no contraindications (Prostate cancer being the most obvious), treatment should be seriously considered when levels drop below 400. Most physicians will disagree and typically go with 300......so sometimes you simply must find a different PCP.
Finally, this post was administered by a physician

This is exceptionally unlikely. I have seen body builders with T-levels over 5000. While this is very unhealthy, having dealt a good bit in this field, "Roid Rage" from therapeutic administration would be EXCEPTIONALLY improbable

5th round for me. 16 pellets this time, have had one more each time from beginning. No rage but definitely " moody" from time to time.

My total testosterone was low 200's before implantation. I get 10 200mg pellets with an arimidex blocker and my total testosterone gets to 800-900 with that and the estradiol level is kept in check with the blocker (I'm a mild converter). I start to feel better within a week. I feel absolutely fantastic from two weeks forward. Peak level occurs roughly 4-6 weeks with a very gradual drop off from there. I can notice a difference between 600-700 and 800 where I seem to feel my best. My doctor does not use testopel, but BHRT pellets that are a higher milligram and seem to work well for me. I think figuring out how many pellets (milligrams of testosterone) you need to get your levels to the point where you feel good is key. While the shots work I never feel as good with them (sense of well being, mood, etc.). My cognitive function, libido, and ability to build muscle from my modest workouts is very good with the pellets. Also very good erectile function. I keep ice on the insertion site and am careful with it for the first few days post implant and have never had a problem or marked discomfort. Hope this helps.

Is it a bad sign that my urologist doesn't know what is a high estradiol level is?(retorical question) Testosterone is 587ttl and 142 free. Estradiol levels not back yet(whole other story.) I gain 15lbs in 3 weeks. 10 in first week. Still feel like crap.
Any recommendations for a good Dr in the southern Chicagoland area. Oh, and
I had to fill my thyroid doc(Endocrinolagist) in about the pellet implementation precedure. Need a new one of them too.

Manny, it sounds like you have found an BHRT informed practitioner! The size and rigidity is key in pellet therapy, allowing for a more sustained level as well as extended time between needing reinsertion. Has your practitioner recommended micronizes DIM? There are multiple reasons for taking the supplement: 1. Helps estrogen metabolize down the healthy pathway 2. Increases your LH, which results in less testicular shrinkage 3. Binds with your SHBG/hormone binding globulin, which prevents your SHBG binding with your free testosterone 4. It is a natural aromitase inhibitor, if you are borderline with an Estrogen ratio of 4-6%, you may not need the RX....but ask your dr about that. Just be careful not to drive estrogen too low! Enjoy the benefits that come from sustained hormonal balance!!!

Is the Estrogen rated against free testo or ttl testo? And do you break it all down to the same measurement? Ng/dL for ttl, pg/mL for free testo, pg/ml for E2.
Testo free: 126 pg/ml
Testo, %free 2.1
SHBG 27 nmol/L
Testo, ttl. 587 ng/do
Levels after 15 Testopels, lab didn't do E2 have to go back. What should a healthy E2 level be for these numbers?
Numbers before implants were: testo free 42 pg/mL
Testo, %Free 1.9
SHBG 27 nmol/L
Testo, ttl 216 ng/do
E2 was 15 pg/mL
Where does that put me as what kind of converting my body does?

Don, just divide your post pelleting T. Test by your post Estradiol and multiply by 100....3-5% is good, 6% is probably treatable with DIM, anything over that you would want to take a rx aromatase inhibitor, but check it before next pelleting, you may can taper down your dose. You just don't want to drive your Est too low, that will make for an angry man...and a T Test at 587 may give you some symptom relief, you recieve the best results at a leevel of 800-1200. I'm not a doctor, but I am in this business. Unfortunately, a lot of doctors are very uninformed in BHRT.
I have an appt this afternoon for my 7th round of biote and have been on this therapy for just under 3 years, I am 53, but feel like 23!!!

Don, unfortunately, the closest Dr we (biote) have that could handle your HRT, including your thyroid is in Ft. Wayne, IN....but we are training 35-50 practitioners a month, so it shouldn't be long before we are closer to your area!

What is the web site address? I will take a look at what you offer. It is a couple hour drive , however a short time in testosterone years to feel better than I am now.
I'm seeing my pcp today and will ask if there are any one else she may recommend.
I'm waiting on my E2 labs.
Your explanation of E2 to Ttl testo ratio was not clear to me. I will look around for something I may understand easier. I have been finding that some suggest Hcg IV's along with Pellet therapy. Your thoughts?

Don,
Sorry for not being clear on the equation. Send me your post total test and Est when you have both and I will calculate it for you. Biotemedical - As far as Hcg, we don't recommend that or HGH and DHEA. Once your hormones are balanced you shouldn't need the others. One thing about DHEA is that it is like a crap shot at converting to what you need.

These are the most recent with 2 weeks between Testosterone and Estradiol
9/12/13
TESTOSTERONE,TOTAL 587 300-890 ng/dL
REPRODUCTIVE HORMONE BNG GLOB 27 11-80 nmol/L
TESTOSTERONE, % FREE 2.1 1.6-2.9 %
TESTOSTERONE FREE,ADULT MALE 126 47-244 pg/mL

9/27/13 ESTRADIOL 37 pg/mL
A urologist has been managing my Testopel implantations. Worries arose when he did not have an answer for me when I asked about estradiol levels. Had no clue where healthy ratios should be but did mention Arimidex.
I have been having what I can only describe as hot flashes(heavy sweating at forehead and sides of nose along with neck) frequently since last implant. 8/6/13
I have had a bunch of other blood work done also if your interested.

Thanks
Don

I just had the testapel's inserted the previous day! I have no discomfort from the insision, and had a partial erection this morning. This is the third time for this procedure. My only complaint is that the dose isan't high enough. The bad thing about it is that I know it works at higher dosing levels. Because the first time I had them impanted I had twelve! And that was almost a perfect dose. Now the Insurance company is involved. I can only get six. I am wondering if the supply is short. My second dose was delayed for about a month. And only recived six. And I am still reciving six. Androjel does work. But I can not keep it on my skin long enough to absorbe. In addition, I run out of skin to apply it for the prescibed dose. And I fear of cross contaminaion. I would hate to wake one morning and find my wife wearing a beard.

Try the IV's. You can do them yourself and you can get the level where you need them. I tried both the pellets and IV and I prefer the IV's. Although with my million dollar deductible insurance I was paying for the pellets in full from my HSA so it was expensive. About $700 for 10
