Where this affects your creatinine level is usually through dehydration. This medication is classified by the FDA as a diuretic, which means it works by removing excess fluid and sodium from the body and if someone isn't taking in enough fluids to make up for it, it can cause dehydration as a side effect.
I actually had that happen with a different diuretic prescribed by my cardiologist a few years ago. She had me stop taking it for a couple weeks and then my levels returned to normal.
Dehydration is a very well known side effect and even listed by the NIH as a possible cause of elevated creatinine levels.
