Based on the description provided, I found your blue and white pill to be Hydrochlorothiazide (12.5 mg). It is manufactured by Cadista.

Hydrochlorothiazide (HCTZ) is often used in the treatment of hypertension, congestive heart failure, symptomatic edema and the prevention of kidney stones.
