Hi anthony,
Although I haven't found a precise identification, I'm almost certain that this pill is manufactured by Novopharm (Teva) Canada, based on the imprint.
I did come across a similar pill from that same manufacturer, describing the pill as follows: "yellow, pentagon-shaped tablet, marked "novo" on one side and "10" on the other side, contains cyclobenzaprine hydrochloride 10 mg".
