Metoprolol And Low Heart Rate - Need Response Quickly

My husband takes 50mg Metoprolol succinate 1 x daily at bedtime for V-tach (no heart disease) along with 20mg lisinopril and 1 baby aspirin. He recently got an Apple watch and was alerted that his heart rate while he slept had dropped to 37 and 34. We recently moved, we have to wait for an appt to get in with a new doctor and then have to be referred to the cardiologist here for a check up as it has been about a year since he last saw the cardiologist (that is when the meoprolol was increased to 50mg) and was told to come back in a year. Is the low heart rate at night an urgent concern or is it ok to wait until he can get in with the new doctor. Or should he cut the tablet in half? Thank you for your help!

Hi Rosan,

Sorry to hear about your husband's situation.

According to wikipedia, "during sleep a slow heartbeat with rates around 40–50 bpm is common and is considered normal."

Those levels of 37 and 34 are alarming to me if it's recurring. On the other hand, if this only happened on one or twice, one might consider it a fluke or malfunction of the Apple watch. In my opinion something else should probably be used instead of (or along with) the Apple watch to ensure that these readings are indeed accurate; just for cross referencing data.

Based on what I conclude from your post, I wouldn't necessarily throw up the alarm bell just yet, until additional data from other devices and/or a doctor's evaluation has been introduced to the scene; but it's probably a good idea to keep a very close eye on things in the meantime.

RE: "Or should he cut the tablet in half?" - I can only speak for myself, but if it were me I would try that option as well. The chances of this issue being a side effect of Metoprolol isn't necessarily out of the realm of possibility either.

Thank you for your reply, makes me feel better to have another opinion. Last night a low heart rate was recorded again early am while he was sleeping, heart rate of 31. I called today and left a message with his cardiologist that he went to before we moved, am waiting to hear back but have decided that if I don't hear back before he takes his meds tonight that we will cut it in half and try taking it 12 hours apart and see how that goes until he can get into a new cardiologist here. I barely slept last night for worrying about him. The fact that it was recorded so low 2 nights in a row is alarming to me. I feel pretty certain that it is due to the Metoprolol and has probably been happening but we were just not aware of it. I am ordering the Kardia band to use with the Apple Watch for cross reference and to give us some peace of mind!

Get off that med. Try Bystolic as a better alternative.

Yes the heart rate is much too low and he should see a cardiologist as soon as he can I take only 25 mgs metoprolol once at night and 10 mg lisenopril mornings and they make me so tired and no energy. I stay very cold with this much med even in summer wear sweater and socks because it makes me cold . You do know that this is a beta blocker which slows your heart rate down ?
