These are specialized ointments ordered by a doctor and filled by a compounding pharmacy, but going by what I've learned, my best guess from what you've listed is that it contains Lidocaine (which is as topical anesthetic), Pregabalin (which is used to treat nerve pain), Cyclobenzaprine (which is a muscle relaxant), and Gabapentin (which is also used to treat nerve pain.)
However, you can always contact the pharmacy to be sure. They should be able to tell you the precise ingredients, in case I'm wrong, as well as why it was prescribed.
