a Dr. prescribes nifedipine 20mg slow release three times a day for a customes is it safe to give him/her 20mg of the immediate release three times a day

I would suggest doing so, without their doctor's approval.

Nifedipine is a calcium channel blocker, which is most commonly used to help treat angina and replacing the time released medication with an immediate release could result in their getting too much of the medication at once, which could cause an overdose.
