Has a doctor or dentist prescribed it for them?

Is it a permanent adult tooth, or a baby tooth?

That is kind of a young age to be given a narcotic, such as the Codeine in this, if it isn't absolutely necessary, so I wouldn't advise giving it to a child, unless a doctor or dentist has specifically said you should.
