Apo Methylphenidate (Top voted first)

I have been diagnosed with ADHD and Bi-polar disease. My Dr. gave me 2 different meds which are Apo-methylphenidate and Sandoz-Methylphenidate. What is the difference between these 2 meds and why are they giving me 2 different ones?

You want Novartis methylphenidate! It is a small round pill, color will depend on dosage, and the pill will be stamped "CIBA".

I too have heard kvk methylphenidate is terrible, as is kvk and teva dextroamphetamine, as is teva and aurobindo mixed amphetamine salts. So many ADHD patients think they don't respond to stimulants, when really they WOULD respond to brand or authorized generic stimulants, but feel terrible (with no symptom relief) from the majority of non authorized generics.

You are 100 percent correct! I went through all of the poor generics you named and quite a few other lousy (non-authorized) generics of both classes of CNS stimulants, to no relief of ADHD symptoms and plenty of negative "side" effects.

After researching what others were saying about non-authorized generics vs brand or AGs, I asked my pharmacist and doctor about the situation. Predictably, I got the whole spiel about how generics only differ slightly, in terms of binders and fillers. I honestly do not see how a difference in inactive ingredients or "plus or minus twenty percent in active ingredients" could create the absolute disparate effects I've experienced, to the extent that, for example, Teva and Aurobindo mixed amphetamine salts didn't even feel like it was a stimulant. And the GI distress caused by these two generics (in someone with a very strong GI system, no less) implies something else going on.

Luckily, my doctor and pharmacist did not stop me from trying brand and authorized generic ADHD medications. The results were a 180 degree difference. I experienced symptom relief and few "side" effects from both dextroamphetamine (brand) and methylphenidate (Authorized generic Novartis as you recommend!).

It still is a hassle when dealing with a new pharmacist who tries to tell me that they are all the same and, sometimes, even that they are "not legally required to order a certain manufacturer or the brand name". I stress out every month when I go to pick up a new prescription. But this is still better than thinking that my ADHD was making me fall asleep an hour after taking generic Adderall (hmm, just like how I react to Sudafed!) and that I had no hope that medication would help me.

They aren't different medications, they are both just Methylphenidate, APO and Sandoz just refers to the manufacturers.

This medication may cause side effects, such as: nausea, headache, dizziness and insomnia.

I have been taking methylin er 20 mg. for 2 years. I went to get a new prescription filled & I was given methylphenidate sr 20 mgs. because they were out of the methylin, but it is not working the same. Is there a difference?

Reggie, Methylphenidate is the active ingredient in Methylin, but it is the generic for it. You have the same medication, in the same dosage, but there can be some fluctuations in the amount of the active ingredient between the name brand and the generic, so that may be what is causing your problem.

Yes! Anyone with ADHD feeling fatigued/nauseated/irritable on your ADHD meds, check to see if it is generic or brand, IR or XR. Both can cause insufficient absorption. There is such a small window of effectiveness for each person, too low dose and too high dose cause similar symptoms, everyone's different, and you get a situation where people give up on treatment without troubleshooting.

If you're struggling, first switch to an IR brand med to pinpoint what will work and avoid pitfalls. (Zenzedi vs brand Ritalin IR). Then figure out if you respond better to (IR brand) amphetamine or (IR brand) methylphenidate, then dial into the right dose. Only then should you try XR/ER vs IR, or experiment with different brand XR/ER enantinomers (Adderall XR/Mydayis vs Vyvanse, or Concerta vs Focalin XR) and only after THAT should you begin dealing with "generics roulette" if you NEED TO (ie, you prefer IR, but also liked Mydayis or Adderall XR the best substantively, which means an IR delivery BUT also a mix of l and d entianomers of amphetamine which only happen to come in a generic, ie generic Adderall IR--other than that I don't see the need for generics unless cost is an issue).I

Once you find your optimal ADHD class/es of medication, delivery method, and brand/chosen generic, you will find it worth all the trials. This method takes the guesswork out and enables you to see patterns within all of the attempts.
