Oxycodone 45

I've been taking 30-40 mg of oxycodone #100 tabs per month for two years. Today my pain management doctor says "I'm not longer writing any pain meds except this, belbuca." He gives me 30 tabs of Tylenol #4 to take for withdrawal while pharmacy orders in new drug. He is starting me on 450 mcg films bid. I am being changed from 50mch every 48 hrs and 40 mg of hydrocodone a day to oxycodone and the doc only gave me 15 mg three times a day ir release and i am hurting bad. I have a feeling this is not even close to what i am use to. What should i do or say to my doc. I am working with the va military docs which are very strict. ## So you were on a Fentanyl patch that you changed every 48 hours and the Hydrocodone and they've now cut you down to 15mgs of Oxycodone 3 times a day? There's really no way this is going to work, you're going from a time release patch of the most potent narcotic available, down to just 45mgs a day of Oxycodone and that's not even taking the additional Hydrocodone into account. I have been taking qty of 120/mo,10-325 (percocet) for 2yrs with no side effects. My new MD reduced the qty to 90 and all was ok. He then reduced the qty to 60/mo 2 weeks ago and now my back pain has increased so much I can no longer continue my 1/2 mile daily walks. On his progressive timetable that qty is to be reduced to 45/mo which will be intolerable. I think I will be ok taking the same strength for 75/mo or would I be better off taking a qty of 60/mo 20-325 extended release? Does anyone know if 2 15mg morphine tabs would be equal to 1 30mg oxycodone. Both are immediate release. I have been on the 30's but due to my upcoming surgery my Dr. feels that I need stronger pain med's. This is a nightmare!! I suffer from multiple auto-immune diseases such as Connective Tissue Disease, R.A, Livedo, Raynauds, & traces of Lupus. All of which cause EXCRUTIATING PAIN!! Say I take 3 30mg oxycodone, how many 30mg instant morphine would I need? ## Oxycodone is one and half times stronger than Morphine, so for each 30mg Oxycodone, you'd need 45mgs of Morphine, or for 90mgs, you'd need 135mgs of Morphine. Both are narcotics, so you should make sure that you aren't taking more than your doctor has prescribed, because they can cause depression of the central nervous system, as well as respiratory depression. I've been taking 45-60mg of oxycodone daily for quite awhile now. My doctor would like me to cut down to 45mg (3 x 15mg) more often and add 10mcg/hour buprenorphine patches and only take 60mg (4 x 15mg) as needed. I've read that buprenorphine patches may cut down the analgesic effect of the oxycodone, have others experienced this? I wonder what the difference is between 15MG MS Contin vs 15 Mg Oxycodone.
