I am a doctor who treats and "enjoys" a chronic pain problem. My PCP suggested nucynta. We tried it for several months, and I tried it on some of my patients. From my experience in both realms I feel confident in stating that nucynta ER is complete crap! It neither helped me, nor anyone I prescribed it for. What's Nucynta ER comparable to as far as a pain reliever goes? ## According to the study linked below from NIH.gov, "Tapentadol IR (the active ingredient in Nucynta) has comparable analgesic efficacy and overall safety to that of Oxycodone IR for the relief of moderate to severe, acute lower back pain as well as leg pain" Ref: ncbi.nlm.nih.gov/pubmed/23703422 Another clinical study also compared 50mg and 75 mg dosages of Tapentadol IR to Oxycodone HCl IR 10 mg and reported similar efficacy, at: ncbi.nlm.nih.gov/pubmed/19302899 I understand that this is referring to immediate release and not extended release, but perhaps this information will at least give you an idea of its potency/capabilities and what level of relief you might come to expect. My doctor put me on nucynta er 150 mg x2 daily for chronic pain and I'm not getting any pain relief. Does it take time to work? I've been on it for 3 weeks. ## Hello, Kurtfen! How are you? No, it is an opiate and should have some effect within about 15 to 20 minutes of taking it. However, it could be that it's just not the right medication to help you. The FDA classifies it as a narcotic analgesic, so it has the potential to be habit forming and may cause side effects, such as nausea, dizziness, drowsiness, dry mouth and constipation. Have you informed your doctor of the issue? You may need to try a different medication.

I have been on morphine er 30 and ir 15 for breath rough pain for 3 years I went to pain management dr today and she switched me to the nucynta 100er, and nucenta 50 for breakthough..does anyone know if this will help my back..I have 3 bulging discs and degenerating disks..I work full time and do alot of lifting and bendin..the morphine had worked great just didn't know if I got use to it. I take these drugs and I am always sleepy. I also have MS, but I am tired about 3 hours after getting up. Why? Is this a possible side effect? ## It could be due to the medications, since they are all listed as causing drowsiness, dizziness, and sedation as side effects, according to FDA reports. However, according to the NIH symptoms list, it could also be due to your MS. Additionally, it might be a combination of all of the medications, as well as the MS. How are you doing, now? My doctor’s office informed me it might become unavailable because of the hurricanes in Puerto Rico, which is where it is manufactured. When I went to refill at the end of January, sure enough, Not Available. I am using Fentanyl 25mg patch for chronic back pain. Is it ok to use nucynta ER 50mg for breakthrough pain? ## Has your doctor prescribed them both? It is unusual to prescribe 2 time released medications, together. That's not usually the way breakthrough pain is handled, usually there is one time released medication and one instant release, or regular release medication, because they help with pain much faster than a time released medication.

I was having issue finding my Dilaudid Rx. I ran out and the pain was unbearable. (I used my Rx for Tramadol 50mg x 4-6 daily to alleviate any withdrawals. It doesn't do much for the pain.) My doc switched me to a new time released med Nucynta ER or Tapentadol. It works great. It comes in 50mg - 250mg. Equivalent for my Dilaudid 4mg x 4 was Nucynta ER 150 mg, 1 q 12 hr. but we had to work up to that dosage through titration. It says not to cut the pill and I have not tried. If you have true pain and a legal doc, try this. It is readily available at CVS, usually the 24 hour stores. I don't wake up in knots and crying in pain. What are the best times to take Nucynta ER? I take it in the morning but it wears off in the afternoon and I don't feel well. ## There are really no best times to take it, it depends on what works for you. If it's wearing off fast, you may need a dosage adjustment, or you might need to try a different medication. The FDA warns that Nucynta carries the risk of being habit forming and may cause side effects, such as nausea, dizziness, headache, constipation, and dry mouth. I've been on the OxyContin 40mg 1 tab 3x day for about 15 yrs. 3 years ago my insurance took it off their Formulary list. 1st Year my doctor got the insurance to approve it. Last year I was put on the Opana which made me sick, throwing up every time I took it. The insurance approved me again to the OxyContin. This year it was denied so I am on the Nucynta ER 200mg 2x's a day. I have had 2 Cervical Fusions leaving me with a plate & 15 screws in my neck. I've had a Reverse Right Shoulder Replacement. Hi there, New to this forum, and appreciate any replies/feedback. My PM doc (who has changed my meds each visit x3) Rx'ed Nucynta 50mg 3x day, a change from Percocet 10/325mg 3x day for BTP. Also switched to Oxycontin 10mg every 12 hours (from Opana ER) Stated he wished to "lighten any burden on my lungs" given my medical status: pulmonary nodules which are increasing in size, both lungs, biopsy next month; severe asthma, residual COPD S/P bilateral PE's 10 years ago. Using Symbicort inhaler. I am uncertain how Nucynta would lighten the burden on my lungs, given the warning about lung conditions/impairment, and specifically asthma--of course, respiratory depression can occur with many pain meds. I've been on 75mcg fentanyl patches for about a year. I started having some issues with my blood sugar and sweating that I thought were from the patch. Then this month I found out my wife had been using them so I spoke to my doctor about changing. On 3/4 he switched me to 50mg nucynta er twice per day. Told me to discontinue the patches immediately. I did and had the left overs destroyed or whatever they do with them. I was fine for about a day and a half. Then started feeling very anxious and nervous. I would note the first night I did have hives on my arm but they went away quickly and I associated them with someone's body spray. Now it's been 5 days and I haven slept in 3. My sister was taking Nucynta and Prednisone for her Rheumatoid Arthritis and it weakened her immune system. She had only been taking these 2 medications about a year. One day she got strep and it killed her. Within a couple of days of feeling sick, she was rushed the ER and passed away. She died of septic shock. I blame the drugs. ## Hello, Diana! How are you doing? I am so very sorry for your loss. Prednisone is a steroid class medication that can cause suppression of the immune system, so yes, it could have contributed to the problem. It can also headache, fluid retention and weight gain. However, not being a doctor, I can't say with certainty that it did. Have they attributed her death to anything? What do they say was the cause?

I have been on this new EXPENSIVE drug EXALGO 16mg, The doctor's around here do not want to prescribe what I can take ( I am allergic to so much) and this stuff makes me sick and I break out in Hives worse then any other medication I have been on! I suffer from chronic pain/neuropathy. I am medically disabled due to the neuropathy and chronic pain. Currently I am taking A total of 400 mg of Nucynta ER daily and a total of 200 mg of the center I are daily. Initially I was taking 200 mg ER in the morning 200 mg at night and 50 mg of IR in the morning and 50 at night. Hi, all, I just moved to Covington, KY from. Brooklyn, NY. I am taking Nucynta ER 150 mg/3 x a day, lyrica, celexa, Xanax 2 mg/3 x a day and a muscle relaxant. The main issues are the Nucynta and Xanax. I have had doctor after doctor laugh in my face when asking for this prescription. I started Xanax five years ago and Nucynta about a year ago. No one wants to prescribe Nucynta, either. I have tried Vicodin, oxy--everything. But finally found that Nucynta (a drug I paid $1500 for WITH insurance in NY) works for me. I have severe joint pain throughout my body. It has been extremely severe these past few months. Unable to drive on bad days. Barely able to walk. Milder attacks allow me to drive and walk w/ a cane due to herniated and broken lower disks. My PM doc has put me on 200mg Nucynta ER 2x daily and 100 mg Nucynta for break through pain.
