My doctor changed my dose from 6 mg extended release to 12 mg extended release. I hope this will help. How much 12 mg will be released in my system at once?? I take 2 every 12 hours. ## Hello, Chris! My doctor told me to up my hydromorphone from 6 mg to 12 mg extended release. I was taking 6 mg extended every 12 hours with 4 mg enstant release every 4 hours. Now I'm taking 12mg extended release every 12 hours with 1mg enstant release every 4 hours. How is that upping my dose?? Wasn't the 6 mg extended release with the 4 mg enstant release every 4 hours more then I'm taking now ?? Please help. ## Hello, Steve! How are you? Yes, you are correct. Adding up the two 6mgs a day and the 6 4mg one comes out to 36mgs daily. But 12 mgs twice a day, plus 1mg 6 times a day is only 30mgs. My sister has a chronic illness and severe chronic pain. Amongst other medications, she takes 12 mg. hydromorph contin three times a day, and also has 3 mg. hydromorph contin for higher pain days. Recently her pain has gotten substantially worse, and one day she opened a capsule of the 12 mg. strength, and noticed that the contents were changed. The capsules used to be filled with small "beads" of medication, but now they appear to be filled with a crystal like substance closely resembling sugar granules, and they are sweet, like sugar. Laws are crazy in FL. I just moved to Tampa from Pinellas County. Because my doc is in Pinellas county, I can't get the medication filled in Tampa. Walgreens won't fill my percription of 180 , 8 mg hydromorphone without contacting doctor to get a code to fill is this true my doctors office was closed so I have to try again tomorrow ## I am also on 8mg of Hydromorphone and have my Rx's filled regularly at Walgreens. The max perscribbed dose of this medication is 4x/day which comes to a monthly script total of 120 tablets. hello i have chronic back pain ive been on a bunch of pills and nothing seems to be working as of now im taking 4 percocets a day and this new thing called kadian 100 mg sustained release just asking what it compares to cuz i was just on hydro morph cotin 24 ml twice a day with 4 percocets just asking if this 100 mg kadian is just like a hydro cotin if it is like a hydro cotin what mg would my kadian 100 mg compare ## This is an old post, but kadian 100 is morphine - sustained release for 12 hrs- and is a very high dose. Hydromorphone is a stronger drug. You must have tolerance to high opioid doses to take Kadian 100 (or overdose is likely). I took it for years, but have gradually tapered down to 30 mg twice daily. It took about 2 yrs to taper that much.

I take 4, 2 mg tablets every 8 hours. I find taking 8 mg at once works better, instead of 4 mg every 4 hours. Everyone is different i guess. I, too, have been having a serious problem finding a pharmacy that will (or can) fill my perscription for 120 dilaudid (hydromorphone) 8mg tablets. I am in the end stages of cervical cancer that has spread too far for treatment. The doctors all say "we will keep you comfortable." My perscription was written 18 days ago and after calling a few pharmacies and realizing that would not get me any answers, I had to have someone physically take me to 24 pharmacies only to be told they don't have them at all or that the ones they get go to their regular customers and that the suppliers are only sending them a very limited quantity each month and they can't even order enough to fill my perscription at all. Unless a patient has a condition that is possibly terminal, such as stage 4 cancer, then the DEA recommends they restrict it to the equivalent of no more than 120mgs of Morphine a day, which would 30mgs of Hydromorphone. If a doctor were going by that, they'd most likely stick with prescribing 3 a day, of the regular release, to be taken every 8 hours, so a supply of 90 tablets for a month. However, some states, such as New York, have further limited that recommendation to no more than the equivalent of 100mgs of Morphine a day, which would be 25mgs of Hydromorphone, in this case, that would still be 3 tablets daily. I take 12 mg hydromorph contin twice a day for chronic pain. Until I get to the doc for a new prescription, I only have 18 mg capsules left... Can I remove some of the granules to reduce the strength until I get to the doc ( probably after the holidays). And, any idea how many I would need to remove? I got a script from my pain mgmt doctor today for 120 4mg Hydromorphone, been doing so for years. Had it filled but got 120 hydrocodone 10/325. Told them that was wrong and they sent me back to doc to get another script. I went back and had it filled only to find i now had both scripts filled and ready for pickup. I just got on medicare and had been given my pain meds which are Hydromorphone 8 mg (120) and morphine sul. 60 mg 2 daily. My prior Dr had me on 100 mg of the morphine and 180 of the hydromorphone. I can see a pain specialist who takes insurance but don't know who.. Can anybody to go to. Can anybody refer me to some Drs that will write a reasonable amount of medicine so that I am not in pain. My wife takes 4-8mg dilaudid a day and is on 125 mcg/h fentanyl patch she is having really bad nausea ## She is taking to much narcotic! She is lucky she isn't overdosed. I am a 100% disabled veteran and have been on pain management for about 13 years. (80MG METHADONE daily... yes for pain, 6mg HYDROMORPHONE daily for break-through pain, 1.5 mg day CLONAZAPAM as sleep aid/anxiety and 1200 mg day GABAPENTIN for servered nerves,.
