Yellow And Purple Capsule Lot2745 Ftn101 Dog Dewormer

The box was thrown away, bought from petsmart - it is a dog dewormer. I need to know if this is right before giving it to my dog... I need to know the dosage amount and name; what type of dewormer it is?
