Is the marking possibly 3604 V?
If so, this tablet is manufactured by Qualitest Pharmaceuticals and they list it as containing 5mgs of Hydrocodone and 325mgs of Acetaminophen.
The FDA lists this as being a narcotic analgesic, so it has the potential to be habit forming and may cause side effects, such as nausea, dizziness, drowsiness, dry mouth and constipation.
It is an effective narcotic for treating moderate to severe pain.
