Actavis Pharmaceuticals reports having a tablet with A over 5 on it that is white, and round, which contains 5mgs of Oxycodone in a regular release formulation. NDC code: 00228-2876

The FDA warns that it carries the risk of being habit forming, and may cause side effects, such as nausea, dizziness, headache, constipation, and dry mouth.
