The markings indicate the pill in description to be Baclofen (20 mg); which is primarily used to treat spasticity.
NOTE: The imprint in the drug database shows TV 4097 and not TL 4097. Also the -20- is replaced by -TV-. It is manufactured by Teva Pharmaceuticals USA.
