I have mixed both but not taken at the same time a few hrs later I may take one and I have had no side effects although I have been on both medications for years so my system maybe used to it.

It is virtually doubling up on the same medication, so you really should stick with one or the other.
Mixing them creates a much easier risk of overdosing, either on the narcotics or the Acetaminophen.
This is a narcotic analgesic, so it has the potential to be habit forming and may cause side effects, such as nausea, dizziness, drowsiness, dry mouth and constipation.
Has your doctor prescribed them both?
