I have started the fentanyl patch. I put my first one on and after 24 hours, I then realized it had two pretty small wrinkles in it and I'm not exactly sure if it is completely effective... Pain isn't quit there but not sure if it that will effect the absorbs ion into the skin.

Two very small wrinkles should not really effect it, there is still enough in contact with your skin for the medication to be absorbed and work.
Fentanyl is a very potent narcotic pain medication.
You may experience side effects such as: nausea, drowsiness, dizziness and constipation.
