Sankyo Pharma lists this as being a Benicar HCT tablet, which contains 25mgs of Hydrochlorothiazide and 40mgs of Olmesartan. It is most commonly used to treat high blood pressure.

The FDA lists its typical side effects as possibly including nausea, dizziness, headache, increased urination, and hypotension.
