This tablet contains 30mgs of Dextromethorphan and 600mgs of Guaifenesin, it is a cold remedy called Guaifenex DM.

What type of rash? Is it just a rash, or is it more like hives that are itchy?

It could be an allergic reaction to this medication and you should see your doctor about it, in case it needs treatment.
