I can name off 2 manufacturers of the 40 mg dose (although I believe they may be getting discontinued). Anyways, they are Mallincrodt Pharmaceuticals and Cebert Pharmaceuticals.

40mg methadone WAFERS not "biscuits" are used in clinics only. I doubt you will be able to find anyone other then a methadone clinic to give them to you.
I think maybe a pharmacist can order them, but will he? Unless you go to a mom and pop drugs store as opposed to Wallgreens, CVS, etc. you won't get them.
