OSM is used to designate it as being osmotic, it's used to make sure that you get a maximum extended/time released formulation of the medication. That's why it costs more, it's not a typical generic, because it doesn't have the usual leeway that they are allowed to have, it is tested more rigorously and adheres to much stricter release requirements.
