A fentanyl patch works without the drowsiness but any drowsiness will taper off with continued use with any medication. Dilaudid is a good medication. Have you tried OxyContin or methodone?
Dilaudid contains the active ingredient Hydromorphone, it is also pretty potent, so it may also cause the high, buzzed feeling.
However, such side effects should start to taper off as her body adjusts to the medication, which usually takes about 4 to 6 weeks. She may also experience drowsiness, nausea and constipation.
You can learn more about the active ingredient here:

Another analgesic she may want to consider is the Fentanyl patch, its time released, so it works over a period of 3 days.

Has her doctor made any other suggestions?
