This is an old post, but kadian 100 is morphine - sustained release for 12 hrs- and is a very high dose. Hydromorphone is a stronger drug. You must have tolerance to high opioid doses to take Kadian 100 (or overdose is likely). I took it for years, but have gradually tapered down to 30 mg twice daily. It took about 2 yrs to taper that much.
