UpdatedDoes 15mg er (of X) mean that the blood level of drug X is maintained @ 15mg over 12hr or does it fluctuate significantly?

That means that it's a total of 15mgs that is released over the given time period, but it is released at a carefully calculated steady rate, as explained by the NIH. So, just a portion of the 15mg dose is released regularly.
