Is 60 Mgs ER MORPHINE twice daily with three 15 mg breakthrough stronger than 45mg of METHADONE

Hi Debbie,

There really isn't a simple yes or no answer for this type of question, so please correct me if I'm wrong in interpreting this information from an opioid conversion chart linked below (under 'Appendix 1'). According to this chart, 120mgs of Morphine ER is said to be equivalent to 48mg of Methadone? If this holds true, that would mean 60mgs of Morphine ER is equal to 24mg of Methadone.

One other important thing to take into account aside from this, is that there are various types of clinical considerations that may affect the translation or conversion of drug equivalencies to & from Methadone (such as a weaker generic, different inert ingredients that affect absorption, etc). Having said that, it seems to me that it would still be imperative to contact your doctor before attempting to make any changes without their prior consent; for your own safety.
