From what I can find, in the U.S. and Canada this requires a prescription from a doctor, which means it would be purchased from your pharmacy, however, I don't have any pricing information.

As to the calories, that will depend on the specific formulation that is prescribed. There are different ones, such as milk based ones, so it would vary depending on which is being used.
