UpdatedNEED J-CODE FOR CARDIZEM

Hello,
I located the following info on a forum, so I can't confirm it's validity, but it appears to be so:
"We have always had to use the J3490 code for Cardizem. We submit the NDC number
along with the description and dosage."
NOTE: I located the NDC# to be 55390-565-05
