The closest match I've seen here by Actavis was a round "pink" pill with a crescent moon on one side and 311 on the other. That particular tablet reportedly contains 60mgs of Morphine sulfate ER.
I think your guess about the manufacturer being Actavis is spot on, due to the crescent moon. However, my opinion is that it would probably be best to contact Actavis directly in order to verify the ingredients.
