My wife was diagnosed with uri due to sinusitis. Why use such a heavy duty antibiotic? So many side effects. It's used on HIV patients and to treat certain STI's. I'm assuming she has neither. Seems I'm missing something. Why not use a safer drug?

It is just an antibiotic that is used to treat various types of bacterial infections, it is no more or less safe than any other, unless someone is allergic to it.
The FDA lists its typical side effects as possibly including nausea, dizziness, headache, non-allergic skin rash and diarrhea.
The antibiotic that a doctor chooses to prescribe depends on the severity of the infection; the specific bacteria, if it was cultured and other factors.
