You should give them the dosage their doctor prescribed. When under 12, it is usually dosed by their weight and according to the severity of the infection.

The FDA classifies this medication as an antibiotic and its typical side effects may include nausea, dizziness, headache, non-allergic skin rash and diarrhea.

The dosage for a 40 pound child could range anywhere from 360mgs a day in divided doses to 540mgs a day in divided doses. I also strongly urge you not to try to diagnose yourself, because improper use of an antibiotic could result in them developing antibiotic resistant bacteria.
