Yes, it requires a prescription from your doctor.
Some of the ingredients you've listed are Amitriptyline, Meloxicam and Tramadol, the topi means its for topical use. I'm not sure what the Pril stands for, though.
But this is a special product used for topical pain relief. A doctor prescribes it and the ratio of how much of each ingredient they want in the cream, then you'd have to take that prescription to a compounding pharmacy and they would specially make it for you.
The FDA lists the typical side effects for such substances as possibly include skin irritation, redness, dryness, itching and rash.
