Really, any doctor that has documented why you're in pain can write one. Though generally, it is expected to come from your PCP, since they are the ones most familiar with your medical history.
Do you currently have a PCP that you see regularly? If so, having them refer you to someone would be the best thing to do.
Can anyone recommend a good doctor in that area?
