Hi Sara,
Because this 'combination medication' does not appear to be available in the U.S. and its individual ingredients ARE available here in the U.S. (by Rx-only), it may be easiest to just talk to your doctor about getting a prescription for each individual ingredient and then taking them together.

Other people may have a way to obtain it from other countries, but that is just my two-cents on the matter.
