The information in this post is correct, but just to add some additional information....

The actual manufacturer of this tablet is Major Pharmaceuticals Inc. and it is no longer limited to just Costco, it is sold throughout the U.S. as the generic in many different stores now. I have found them available as the generic at Kroger, QFC, Fred Meyer, Walgreens and Bartell's.
