Topical--gabapentin,diclofenac,baclo2

I AM USING A TOPICAL MIXTURE FOR SEVERE SHOULDER PAIN (shoulders should e replaced) but it doesn't do much. What could I safely use with a patch?

My friend has either hand that will swell and has extreme pain with this including his fingers , wrist and inner forearm. He knows when this is coming on because there will be tingling in his thumbs and he knows this is starting up again. His fingers get so bad He can't use his hand at all. Any suggestions welcome.. Please try to help with any info. Thank
