I was actually able to find a physician locator for doctors who are known for their expertise in buprenorphine treatment. This physcian locator service appears to be government run, so the contact information listed should be up to date and accurate:
buprenorphine.samhsa.gov/bwns_locator/index.html
According to the FDA, Suboxone is the formulation used in the majority of patients. However I'm sure your medical records will be able to speak for themselves, since Suboxone hasn't been working for you.
