Switching From Oxycontin To Dilaudid Pain Relief

I'm in the process of switching my pain meds with doctors care, i was taking 80mg oxy twice daily with 40mg in afternoon that's total of 200mg a day with 5mg for breakthroughs pain every 4hrs. Doctor starting me out at 2mg dilaudid for break thru and reducing oxy , It doesn't seem to work as well, Can someone tell me how much dilaudid I would need to take as 12hr relief, along with 4mg or 8mg for breakthrough. What is breakdown for 200mg of oxy to dilaudid?

It would take abour 75mgs of the Dilaudid (Hydromorphone) to equal the 200mgs of Oxycodone, per day.

Starting you at 2mgs was way too low, it seems. Have you talked to your doctor about it, again?

I will see him in 1week , at that point I will switch completely to Dilaudid. So maybe start out with 25mg twice daily, and 4 or 8 mg for break thru, does that sound right?

Each mg of dilaudid continue is as strong as they get. I would suggest 24mg three times a day. Dialaudid is five times stronger than morphine. Anymore than I told you and you should ask for a pain pump.

Waiting for months for a reply and it doesnt even pertain to your dilema....BTW--did u ever get a bump from Doc Himself????

I was on 110mg twice dailly (Oxycontin) and my doc put me on 90mg twice dailyof Dilaudid. It is nowhere near the same. You nshould be on a higher dose for sure!!!
