My father (63), PD patient for around 20 years. We always had hard time adjusting a perfect dosage for him.
# Right now he is in 8 sinemet (25/100) pills 4 hrs apart. One every dose he takes 1 regular and 1 CR
Problem: the only good onstate he gets is after first dose. But in the afternoon most of the time the meda dont work
Other: he takes ropinirole (.25) 3 times and Amantadine (100) 2 times
Need help,
1. Can i increase hos dosage?
2. Any other meds can improve Sinemet effectiveness?

You shouldn't adjust the dosage of any of his medications, without his doctor's approval.
What else is different in the afternoon that could be affecting it? Is he more active at that time of day? Less active? Does he eat more? Does he take his medications with food or on any empty stomach? There are various things that may affect how well a medication works, as detailed by the NIH.
As reported by the FDA, this medication can cause some twitches, tics and repetitive movements as side effects, so he may also be experiencing those.
