Sankyo Pharma lists this as being a Benicar tablet, which contains 25mgs of Hydrochlorothiazide, and 40mgs of Olmesartan, it is most commonly used to treat high blood pressure, and certain cardiac conditions.

The FDA lists the typical side effects as possibly including nausea, dizziness, headache, increased urination, and hypotension.
