It is common with some medications to start someone on one certain dosage that is known to be well tolerated, then go up to the next, once it has been ascertained that the person suffered no serious ill effects from the initial dosage.

This is an anticoagulant, so it is completely normal for a doctor to proceed with caution, especially since they have to watch for any signs of bleeding.
How long will she be on the 15mg, before going up?
