Peak And Trough Results. Wtf?

Currently on 60 mg's of methadone. Just diagnosed with epilepsy. 60 mg was just fine prior to the new meds. Then I started the worst withdrawal symptoms. The dr. says that some people just take a while to get use to the new medication. The biggest and worst being Dilantin (300mg) morning and (300mg) at night. After 3 days of hell I paid for my own peak and trough. The trough was less than 20ng/ml. 4 hours the peak came in at less than 20ng/ml again. The doctors were absolutely baffled. They really only said all I can do is up my methadone which would defeat everything I have worked for. Any suggestions or does any of this make sense

I am very sorry about what you're going through, but I can explain it for you…

Dilantin contains the active ingredient Phenytoin and it decreases the level/efficacy of Methadone by affecting hepatic/intestinal enzyme CYP3A4 and it is known to be a significant issue, so you're not getting as much of a dose of your Methadone as you were, before you started the Dilantin. Thus, your dosage most likely does need to be raised.
