It's kind of difficult to say given the limited information in your post.
How tolerant is he to opiates? Has he been taking them for a long time?

When it comes to narcotics, a lot of it depends on the person's body and tolerance to them. Someone that's used to taking them regularly can tolerate much higher doses safely, than someone that rarely takes them.
The best suggestion I can make is to call his doctor and tell them that the current dose isn't helping.
