My Mother Age 79 Is A Hfref Patent And She Is Experiencing Itching

Medicine List: Pan 40 & Thronorm 88mg in the morning before breakfast. Then at 10 am one unit of Lasix 40. Then after lunch ondero 5mg & valent 80 at 3pm. Then another Lasix 40 at night (after food), one valent 80 & one clopitab A75. & at 10 pm aztor 20.

What may be the problem? She eats almost boiled food (non-veg) three times daily & at lunch she takes fish oil. Please help.
