Moved From Texas To Pennsylvania And Tx Clinic Refuses For Forward Paperwork

I left TX after a bad divorce. I have been on Methadone maintenance almost 6 years. I went to a clinic in Huntsville 1 year then a clinic in Houston about 4.5 years. I had one positive U/A about 1.5 years ago but it was wrong. They have a high turn around rate and a couple of people were fired for drug use. A new nurse, I never had seen before and did not know her name. when I came out she said she would hold my urine. I told her we always keep with it until we can seal it, then sign over it. She demanded she hold it. I was NOT happy. After that I had to wait for my dose and my 3 week take homes. (It was 3 weeks instead of 4 because I was at 170 mg and you must be 150 for 4 week take homes) After that 10 or 15 minutes I walked out and she was still standing in the hallway holding my urine. There is no way she could remember my name as she had a bunch of people she had just met. I knew there would be something wrong. It came back positive for coke and Opioid s.. I have no history of coke and I was not a H user, I became addicted from a doctor prescribing morphine and Hydrocodone. i wouldn't take opioid because they are blocked in my brain. that would be a waste of money and time. After 5 years on Methadone , opioids do nothing. I would not sign the paper because it was not my urine. she said I had to come in bi-weekly for 2 weeks., so I don't think she believed me. anyway, I needed paperwork to transfer to Germany for a couple of months before moving to Pennsylvania. I showed the counselor the paper and was standing over the table to tell her what to write as she cannot read German and I can. She screamed at me to sit down, twice. I did and naturally on my paper work it was all wrong. basically she wrote down the equivalent of 1-800-doctors because she would not let me stand up and show her. I didn't notice it until I was on my way to the airport. I stopped at the clinic and highlighted the correct address and crossed out what not to write. She STILL wrote it wrong but I was in a hurry to catch the plane and I thought there is NO WAY she could get it wrong. But she did. When I got to Germany, fortunatelly I had sent the doctor the paperwork from before.. He thought the woman at the Texas Treatment Center was an idiot. I wrote a quick letter to the clinic addressed to the Supervisor and told on her.. when I came back to the US 2 months later the clinic requested paperwork and the woman in Texas will not send what they need they have tried to contact her but she is angry because I turned her in. I called her twice and she said she has sent what Texas requires. all she has to do is write when I started there and when I left. she knows I have to start back at everyday and have been doing so for 2 months. I had a great life before. I even forgot I was on the methadone. I would take my meds in the morning and go to work and go about my day and never once think about the problem. Now, I cannot get a job because i all I can do is go to the clinic everyday. I need to turn her in. Who do I write to in Texas that governs the Methadone Clinics to let them know that the woman at the Texas Treatment Center in Houston refuses to forward my papers?

Hello, Karen! How are you? Sorry about everything that you've been through.

And I'm sorry, but I don't know who you could report this to as I don't know what the central governing board would be.

You might be able to find out via the FDA, you can call and ask, their information number is 888-INFO-FDA (888-463-6332)

If that is the clinic on 34th street good luck. They went from a great clinic to a lousy one in a months time. I went there until 7 months ago when they started watching people pee. They owner has health issues and hired a company to manage it. Search NOMA and go to there website. They will tell you were to file against them.
