need a j code for authorization of lyrica

From the listings I am finding, the relevant code should be J3490.

Lyrica contains the active ingredient Pregabalin, which is an anticonvulsant that is also used to treat certain types of nerve pain.

Side effects may include: nausea, dizziness, dry mouth and weight changes.
